The Realmgate

The Realmgate (also spelled Realm Gate) is a feature found on the Atlas that consumes certain map fragments to open portals to endgame variants of encounters, often containing a pinnacle boss.
It is always located near the starting area of the Atlas, usually directly east of the Ziggurat Refuge icon. It appears as a large circular portal decorated with triangle and gear-like motifs.
List of map fragments usable at the Realmgate

Twisted Domain | Travel to the Twisted Domain by using this item at the Realm Gate. Can only be used once. |
 Simulacrum |
The Simulacrum | Open a Delirium Portal by using this item at the Realm Gate. Can only be used once. |
 An Audience with the King |
Crux of Nothingness | Travel to the Crux of Nothingness by using this item at the Realm Gate. Can only be used once. |
Although similar, 
Expedition Logbooks must be used through Dannig, rather than at the Realmgate.
