While in the fog, Delirium monsters may spawn, and existing monsters deal more damage and take less damage based on the absolute distance from the starting point. The longer and farther the fog travels, the higher the Delirium. Additionally, rare and unique monsters may gain Delirium modifiers, often manifesting as arena hazards or mirages that attack the player or granting benefits to monsters.
Defeating monsters builds up the reward counter that appears near the life globe; after the Delirium encounter ends by leaving the fog for over 5 seconds or manually ending the Delirium, the accumulated rewards will drop in the form of Distilled Emotions (1 random type per reward icon) and Simulacrum Splinters. The quantity of Simulacrum Splinters dropped scales with map tier.

Delirium bosses (Omniphobia, Fear Manifest and Kosis, The Revelation) can sometimes appear in the Delirium fog. Like other Delirium monsters, they must be defeated before the fog dissipates or they will disappear without leaving items. If defeated successfully, they will drop a larger stack of Simulacrum Splinters.
Fracturing Mirrors

Fracturing Mirrors are structures that appear as spires of glass emitting pulses of light that are found in the Delirium fog. They shatter when players approach, spawning packs of Delirium monsters.
Instilled maps
By instilling a map with up to 3 Distilled Emotions, it is possible to add an enchantment that also causes a permanent Delirium fog to persist throughout the entire map. The Deliriousness is determined by the combination of Distilled Emotions and is a static value.
When Delirium is added this way, there is no reward counter and it is not possible to end the Delirium fog. Distilled Emotions cannot be obtained in these kinds of maps. However, Delirium monsters can still be encountered and will drop Simulacrum Splinters directly.
Delirium scaling
The exact effects and scaling of Deliriousness are currently unknown. However, it appears to grant a scaling amount of increased damage, less damage taken, and extra item rarity to monsters.
Delirium scales up to the following percentages:
- An unspecified amount in Delirium mirror maps scaling with distance (likely up to 100%)
- Up to 150% in maps instilled with 3x
Distilled Isolation
- 5%-75% in the Simulacrum at Difficulty 0, up to 45%-115% at Difficulty 4
