Pinnacle encounter
A pinnacle encounter represents the most difficult and rewarding content relating to a given map mechanic. They general feature a difficult boss. All pinnacle encounters can drop exclusive unique items.

Domain with maddened agony and anger.Travel to the Twisted Domain by using this item at The Realmgate. Can only be used once.
can be used with The Realmgate to open portals to the breach domain, the pinnacle Breach encounter. The area is composed of one massive breach. Once the breach is activated, a timer will start ticking down. Killing monsters will extend the timer, and if it reaches zero the breach ends. If the exile prolongs the breach to the end of the area, they may fight the boss, Xesht, We Who Are One.
